There are four forms to be signed after our 4-way zoom meeting with both Disclosure Guides and the couple. These forms should be completed if you are ready to fully commit to working with both professionals. This paperwork ensures we can communicate about the disclosure case and process. It is necessary for discussing timelines, the disclosure document, and collaboration.

For couple: Disclosure/polygraph Agreement

This will be signed by the couple. This is an agreement understanding and agreeing to the pro, cons and possible outcomes of a disclosure.
